Your health information is collected through our EMRs, website, and various intake forms by all custodians and affiliates at Lakeview Physiotherapy Clinic & Movement Studio under the authority of Section 20(2) of the Health Information Act and for purposes identified in Section 27 of the Act, including:

• Providing health services
• Determining or verifying eligibility to receive a health service
• Conducting investigations, discipline proceedings, practice visits or inspections relating to the members of a health profession or health discipline
• Providing for health services provider education
• Carrying out any purpose authorized by an enactment of Alberta or Canada
• For internal management purposes, including planning, resource allocation, policy development, quality improvement, monitoring, audit, evaluation, reporting, obtaining or processing payment for health services and human resource management

When collecting individually identifying health information, the custodians and their affiliates at Lakeview Physiotherapy Clinic & Movement Studio will collect information from the individual who is the subject of the information unless the individual is unable to provide information or authorizes collection from a different source. The custodians and their affiliates at Lakeview Physiotherapy Clinic & Movement Studio will collect the least amount of individually identifying health information necessary to fulfill the purposes identified.

In some circumstances, the health information collected by the custodians and their affiliates at Lakeview Physiotherapy Clinic & Movement Studio through Jane App, may be entered into and processed by Jane AI Scribe for the purposes of assisting with medical record recording and transcription.

The confidentiality and privacy of the health information collected by custodians and their affiliates at Lakeview Physiotherapy Clinic & Movement Studio is protected by the provisions of the Health Information Act and measures put in place by Lakeview Physiotherapy Clinic & Movement Studio’s lead custodian.
